Abstract
El objetivo del presente estudio es el análisis,
a través de la exposición de un caso clínico con
fobia a atragantarse y miedo a padecer cáncer,
del diagnóstico diferencial y del tratamiento
cognitivo-conductual mediante perfiles clínicos.
Método: Se presenta el caso de un hombre
ingresado en salud mental por un cuadro de
desnutrición severa secundario a baja ingesta.
Resultados: Incremento notable del peso,
disminución de la creencia de padecer un
cáncer y exposición a la situación ansiógena
de comer. Conclusiones: En nuestro caso el
componente fóbico es clave en el perfil clínico,
dado que desde la fobia a atragantarse articula
la creencia de padecer un cáncer, de ahí que las
técnicas conductuales obtuvieran resultados
positivos. No obstante, es necesaria una mayor
investigación acerca de la posibilidad de definir
diferentes perfiles clínicos de los pacientes con
hipocondría según prepondere un componente
fóbico o un componente de convicción de
enfermedad.
The aim of the present study is to analyze, through the presentation of a clinical case of choking phobia and fear of getting cancer, the differential diagnosis and cognitive-behavioral treatment in clinical profiles. Method: In the case of a man admitted to mental health by a clinical profile of severe secondary malnutrition to low intake is presented. Results: Weight increase significantly, decreased belief of gettinng cancer and exposure to anxiogenic eating situation. Conclusions: In our case the phobic component is key to the clinical profile, as from choking phobia articulates the belief of developing cancer, hence behavioral techniques obtain positive results. However, further research on the possibility to define different clinical profiles of patients with hypochondriasis according preponderation phobic component or a component of disease conviction is necessary.
